Clinical and administrative systems

Healthcare EHR and legacy system modernization where privacy shapes every choice

Clinical, billing and administrative systems modernized in small steps, with your privacy, security and compliance teams in control of data access and sign-off.

  • Privacy rules decide who sees which data
  • Interfaces and data mapped before anything moves
  • Assessment and a written roadmap come first

What does healthcare legacy modernization cover?

Healthcare organizations run a mix: a commercial or home-grown EHR, practice management and billing, lab and imaging interfaces, scheduling, reporting databases and in-house tools written years ago. Many sit on mainframe, AS/400 or older Java and .NET stacks, linked by HL7 v2 messages, flat files and point-to-point feeds.

A commercial EHR is configured and upgraded through its vendor. Our scope is typically what surrounds it and what runs on older platforms: custom applications, interfaces, billing and scheduling code, and databases. Typical moves: a custom application to a supported platform, brittle interfaces to managed integration, or a database to a new engine. See database modernization and cloud migration.

These systems hold protected health information, so privacy comes first: who can see data, where it can be copied and what is logged. Which laws and policies apply to you, and what they require, is for your privacy officer, counsel and auditors. We follow the controls you set. We do not claim any certification and we do not give legal advice.

How we run it when patient data is involved

Data handling is agreed before technical work starts.

  1. Agree data handling rules first

Sign a mutual NDA, then settle with your privacy and security teams where work happens, who gets access, and whether we use synthetic, masked or de-identified data.

  2. Map systems, interfaces and data

List applications, HL7 and file interfaces, databases and reports, and mark which touch patient data.

  3. Record current behavior

Write tests from synthetic cases for billing, scheduling and interface transformations.

  4. Pilot a low-risk slice

Start with an administrative or reporting function before any clinical workflow.

  5. Phase the rest with clinical sign-off

Each phase has agreed downtime procedures, review by the people who use the system, and a rollback point.

Wrap, move, replace or keep

Often suitsOften does not
Wrap with FHIR or REST APIs Stable system, new apps need dataSystem is unsupported or failing
Replatform the database License or hardware is the issueSchema has hidden dependencies
Replace point-to-point feeds Links keep failing silentlyFew interfaces, all stable
Rebuild a custom tool Workflow is changing anywayStaff rely on exact old screens
Retain and monitor Low risk and little changeVendor ends support soon

Problems we are usually asked about

  • Interfaces nobody owns

Hand-built HL7 or file feeds fail quietly, and one person knows how they work.

  • Reports on the live database

Heavy queries slow systems that clinicians depend on.

  • Unsupported platforms

An operating system, database or runtime is past vendor support.

  • Gaps in audit trails

It is difficult to answer who accessed which record and when.

  • Rules buried in billing code

Scheduling and billing logic lives in code with little documentation.

  • Awkward downtime windows

Upgrades need outages that clinical units cannot easily absorb.

Risks and limits

Clinical workflow changes need your clinical governance. We do not make clinical decisions.

Every extra copy of patient data is extra exposure, so we keep copies few and keep them inside your environment. Migration also exposes data quality problems such as duplicate records and mismatched identifiers, and fixing them is a decision for your data owners.

Clinical units need procedures for when a system is unavailable. We plan cutovers with your operations team, but we cannot promise there will be no interruption. We also cannot promise timelines, savings or outcomes before an assessment.

Questions from healthcare IT and privacy teams

Will you handle protected health information?

Only if your policies allow it, and only inside your environment. We prefer synthetic or masked data. When real data is needed, access follows your controls and a mutual NDA, and you decide who and for how long. Ask your counsel which further agreements your organization requires. We do not claim any certification.

Can you replace our EHR?

A commercial EHR is normally replaced or upgraded through its vendor, and that is a different kind of program. Our work usually targets older custom systems, interfaces and databases around it. If a replacement is under discussion, the assessment can map what depends on the current system.

Can we stay online during migration?

We design for side-by-side running and agree rollback points, so most changes do not require taking a system down for long. We do not promise zero downtime. The real answer depends on your cutover windows and downtime procedures, which we plan with your operations team.

Do you work with HL7 and FHIR?

Yes, as part of interface work. Many organizations put a FHIR or REST layer in front of an older system so newer applications can read data without touching the core. Whether that fits depends on the system and on what your vendor supports.

How does an engagement start?

With the request form or WhatsApp, then a written scope that you approve before any work begins. There is no public price list, because systems and privacy requirements differ. A legacy code audit is the usual first piece of work, and it ends in a written roadmap.

Tell us which healthcare systems are in scope

We reply with questions and a proposed scope, and a mutual NDA can be signed first. WhatsApp is at +92 326 5200333.

What to send

  • Systems in scope and what they do
  • Platforms, languages and interface types
  • Whether patient data is involved in the work
  • Target environment, if chosen
  • Downtime limits and freeze periods

Please do not send patient data, credentials or source code in chat or in the form.

Describe your healthcare systems to us

Send the request form or message us on WhatsApp. A mutual NDA can be signed before you share anything.
